Options for completing your survey
You have a life — we know! That’s why we try to make it as easy and convenient as possible for you to complete your surveys.
Smartphone, tablet, or computer
Use the new myLTFU study portal.
Just mail it back to us in the postage-paid envelope. If you received a paper version of the survey and have lost it, please call our toll-free number at 1-800-775-2167 or email us at LTFU@stjude.org and we’ll gladly send you a new copy.

If you would like to answer the survey questions over the phone, any day of the week, with one of our trained telephone interviewers, contact us to arrange a convenient time:

The telephone survey staff is available to conduct surveys during the following hours:
- Monday-Thursday 8:30 a.m.-9 p.m. CST
- Friday 10:30 a.m.-7 p.m.

- Sunday 12:30 p.m. - 9 p.m.

Participant tips
We asked some of our participants to share their strategies and suggestions for completing their surveys. Here's what they told us:
- "When I get a new survey in the mail, I put it in my briefcase or knapsack so I can work on it whenever I have short periods of free time, like during my commute."
- "I put it with my bills. When it's time to pay bills, I fill out the survey and take everything and mail it at once. Cross that off my list!"
- "My phone interviewer was willing to call me several days in a row while I ate my lunch. It was so much easier for me to do it in short sections than trying to find time to do it all at once."
- "I like doing it online. I'll answer some questions while dinner is cooking. I can stop whenever I need to, and my answers are all saved. When I log back in, it takes me right to where I left off."
- "I was surprised that the latest survey was so easy to take on my phone. It’s easy to read, and easy to select my answers."
