Every two to three years, surveys are sent to LTFU study participants asking them to update their health histories. Participants are given three options for completing the survey.
Option 1 - By Mail
Participants can complete the paper version of the survey and return it in the prepaid, self-addressed envelope. If you have received a paper version of the survey but have lost it and would prefer to complete the survey using this option, please call the toll-free number below to request another survey be sent to you.
Option 2 - Online
Participants can also complete the questionnaire using our secure online surveys, which are password protected for each participant. Current surveys can be found to the right under Related Information. If you have not completed your survey and need your password to complete the survey online, please call the toll-free number below.
Option 3 - By Interviewer
Participants can complete the survey via the telephone with one of the Long Term Follow Up Study telephone interviewers. If you have not completed the 2007 survey yet and would like to complete the survey over the phone with one of the telephone survey interviewers, please call 901-595-6600 or the toll-free number below. The telephone survey staff is available to conduct surveys during the following hours: M-F 8:30 a.m. - 9:00 p.m. CST; Sat 11:00 am - 7:00 p.m. CST and Sun 2:00 p.m. - 9:00 p.m. CST.
